Weight Loss Surgery
Attending a group information session is the first step in your weight-loss surgery journey. Learn what steps follow.
Before any surgical procedure, several steps must be completed to ensure you understand the entire process involved and to determine your eligibility.
The first step for patients interested in weight loss surgery is to attend an informational seminar in-person or online. Our free informational seminars are a great way to learn more about the options and procedures available at Bristol Hospital, have your questions answered by bariatric surgeons and hear from our team who will explain the steps involved in determining if you are eligible for surgery.
The next step is to schedule an appointment with our bariatric program coordinator for an intake appointment. Please call 860.585.3339 to set up your appointment. Please bring your completed intake forms with you. At the appointment, the program coordinator will review your forms with you and answer any follow-up questions you may have after completing your mandatory seminar (Step 1). When you leave this appointment you should have a better idea if you qualify for the weight loss surgery program and what lies ahead.
You will need to complete a number of forms, and bring them with you to your first appointment. If your paperwork is not completed by your first meeting, your appointment will need to be rescheduled. Fill out the forms at the links below and please bring copies with you to you consultation.

Following your consultation with the program coordinator she will help you schedule an appointment with a registered dietitian who will review your medical history, current eating habits and personal wellness goals, and then develop an individualized nutrition plan. Our goal is to help you make gradual, long-term changes in your eating habits and lifestyle, providing encouraging support and advice along the way.
Following your review with our registered dietitian, you will need to make an appointment for a surgical consult. Your surgeon will talk about your goals and go over what procedure is best for you. Our coordinator will help you schedule this appointment as well as answer any questions you may have heading into the surgical consult.
After a surgical consult and meeting with your assigned registered dietitian you will need the following medical clearances:
All patients are required by the program to complete at least one group meeting before having weight loss surgery and are urged to attend at least two (2) meetings after surgery. Research indicates that individuals who regularly attend support group meetings are more likely to be successful after surgery than those who don’t attend. Support and continued education is key to success.
