Who Is a Primary Care Provider?

Because primary care is the first line of care and spans your entire life, there are different types of providers who offer primary care, including physicians, nurse practitioners and advanced practice registered nurses.

Primary care specialists are typically trained in internal medicine or family medicine and can specialize in treating adults, children or both. Pediatricians are providers who specialize in primary care for children. Since gender, age and health conditions can impact care, some patients consider their obstetricians, cardiologists or geriatric specialists to be their primary care providers.

Treating Chronic Conditions

One of the most common things primary care providers do is to help patients manage chronic conditions such as diabetes, asthma or heart disease. Treatment of chronic conditions can be complex and ongoing. Patients may have more than one chronic condition and may be seeing multiple providers, and primary care providers help coordinate the overall approach. .

At Bristol Health, our providers are experts in helping you manage your overall health, including chronic conditions. Bristol Health Medical Group is nationally recognized as a Patient-Centered Medical Home by the National Committee on Quality Assurance. This model of care emphasizes putting patient needs at the center, collaboration among providers and continuous quality improvement.
