Hospice Care

Bristol Health Hospice Care is a specialized healthcare program focused on patient comfort, emotional and spiritual support, and improving overall quality of life. Our multidisciplinary team provides hospice services in the patient's home or, if appropriate, a similar homelike setting such as assisted living, a skilled nursing facility or Bristol Hospital.

We are also dedicated to supporting family and friends in their experience of caregiving.

What is Hospice?

Hospice is a specialized healthcare program dedicated to providing care and support to you or your loved one suffering from life-threatening illnesses. Hospice care is primarily provided to persons at home but is also available in skilled facilities or, if medically appropriate, in the hospital. By providing relief of trouble symptoms and giving social support for the family, our team takes appropriate measures to ensure a comfortable death experience is had for not only the patient but their loved ones as well.

A person using hospice care has a typical life expectancy of 6 months or less.

Services Provided by Hospice

Hospice services are provided by a medically supervised team of professionals and volunteers. The hospice team works together with you to care for your physical, psychological, social and spiritual needs. You may utilize all or part of the available services:

  • Medical care focused on providing comfort,
  • Pain management and symptom control program,
  • Psychosocial and spiritual support for the patient and family,
  • Bereavement support for family members and significant others,
  • Hospice volunteers are available for socialization and emotional support,
  • Registered nurses, social workers, therapists, homemaker/aide services.

Coverage of Services

Medicare, Medicaid and many other insurance plans cover hospice services. Events such as Lights of Love are used to assist Hospice and Special Touch patients as needed. Donations are used to directly support services for patients who are uninsured or under-insured.

Bereavement Program

Hospice care continues into the bereavement period with support for your family. Hospice offers a Bereavement Support Program to help the bereaved realize that they are not alone in their grief and to support them through the grieving process by providing the following:

  • A comprehensive packet of bereavement information and resources,
  • Follow-up correspondence at specific intervals throughout the first year after a loss,
  • Short-term grief support via telephone or office visit (assessment and outside professional referral if needed),
  • Referral to the "Coping with Grief" support group and other community resources.
